What is Gender-Affirming Breast Augmentation?
Gender-Affirming Breast Augmentation is a surgical procedure that helps people align their physical appearance with their gender identity. The surgery typically involves placing implants under the skin in the chest area to create a more feminised appearance, and may also include revision surgery to enhance or refine previous results. It is a transformative, life-changing procedure that has been proven to alleviate gender dysphoria in those who undergo it.
At Pall Mall, we understand the deeply personal nature of gender-affirming procedures and the positive impact they can have on the health and wellbeing of people who undergo them. We are dedicated to providing compassionate, personalised care, and we have experts on hand to support you every step of the way. Your surgery will take place at our modern, state-of-the-art private hospital in Newton-le-Willows, where you will be cared for by specialists with years of experience in gender-affirming healthcare.
Breast Implants
The most common method of gender-affirming breast augmentation involves placing silicone or saline implants beneath the chest muscle to enhance breast size and shape. Our surgeons work closely with you to choose the right:
Implant size and shape: carefully matched to your body type and goals
Implant placement: either under or over the pectoral muscle, depending on how much natural breast tissue you have
Incision location: incisions are typically made under the breast crease or around the areola for minimal scarring
Revision Surgery
If you have had breast augmentation elsewhere and are unhappy with the results, experiencing complications, or simply wish to refine your look, revision surgery may be an option. Common reasons for breast augmentation revision include:
Implant size change: adjusting implant size for a more natural or proportionate look
Capsular contracture correction: removing or replacing implants to address hardened scar tissue
Implant displacement or asymmetry: correcting misaligned or uneven breast implants
Implant rupture or deflation: replacing damaged implants.
Rippling or visible edges: refining results for a smoother, more natural breast contour
Change implant material: Choosing the right implant material - saline or silicone - based on personal preference and comfort

What’s involved with the procedure?
Gender-Affirming Breast Augmentation is a significant procedure that involves a lot of planning and patient care. We’ll be on hand at every stage to ensure your procedure goes smoothly and to answer any questions or concerns you may have along the way. You can find out more about what’s involved at each stage of the procedure below.
Pre-op
Before your surgery, we’ll ask you to attend some consultations so we can check you meet all the necessary medical requirements and so you have all the details you need to make an informed decision about proceeding.
The first consultation will be a virtual one with our dedicated gender surgery Patient Adviser, Mamuna, who will ask for some further information - including your referral letter - and make sure that you’re happy to move forward with the procedure.
The next step is a face-to-face consultation with your surgeon, Mr. Prasad. This will give you a chance to discuss your gender affirmation goals and ask any questions you have about the surgery.
Mr. Prasad will also examine your chest and discuss implant sizing with you. It is helpful if you can show your chest for a few seconds during this consultation, but if you’re not comfortable doing so, you will be asked some questions to describe your chest instead.
Following your surgeon consultation, you’ll have a two-week cooling off period in which you have time to decide whether you want to proceed.
If you’re happy to move forward with the surgery, we’ll arrange a pre-operative appointment with a nurse, at which point we’ll take payment for the surgery, and then we’ll schedule the procedure for a date that’s convenient for you.
The day of your surgery
Your surgery will be scheduled on a date up to six weeks after your initial consultation, and once all the pre-op stages have been completed. On the day of your surgery, our friendly team will welcome you to our hospital, where you will be shown to your own private room to prepare for the procedure.
Your surgeon will visit you before the operation to check in and get your final consent to proceed. You will then meet with the Consultant Anaesthetist to agree on a plan for your pain control and ask any relevant questions.
The surgery will take around two hours and once it’s complete you will be returned to your room to come round from the anaesthetic. Dressings will be applied to your chest and you will be fitted with a post-surgery compression bra, which you will need to wear for at least six weeks to aid recovery.
Most of our patients return home the same day, but we would recommend packing an overnight bag in case you need to stay in hospital. You will be unable to drive for a week after your surgery, so you will need to arrange transportation home from the hospital.
Post-op
After your surgery, we will give you all the information you need to manage your recovery and ensure the best possible results from the procedure. We would advise you to ask any questions you may have about your recovery while you’re in hospital, and we will also give you details for contacting us out of hours in the event of any urgent concerns.
Following your surgery, you may be provided with medicines to take home. Please take these medicines as prescribed, and continue taking any pain relief or HRT medicines as prescribed.

What type of implants are used for Gender-Affirming Top Surgery?
We offer a choice between saline and silicone implants. Saline implants are filled with a sterile saline solution and silicone implants are filled with a silicone gel that replicates the feel of natural breast tissue. Our surgeons will discuss the size, shape, and material of implants with you and advise which may be best for your specific goals during your pre-op consultation.
Will this surgery leave scars?
Yes, you will have permanent scars after this procedure. The scars will fade over time and their visibility can be reduced with proper scar care.
Are there any risks involved with his surgery?
Breast Augmentation is an invasive procedure and as such does carry the possibility of complications. Some swelling and bruising is expected, but there is also the risk, in rare cases, of bleeding, infection, implant rupture or leakage, or loss of sensitivity in the chest area.
If you have any concerns about the potential risks or you’re experiencing any worrying pain or discomfort after the surgery, get in touch with our team via the contact details provided so we can help.
What is the recovery process like?
Recovery can vary for each individual, but in all cases it’s important to follow your surgeon’s advice to aid the recovery process.
You can usually expect to return to work and light activities 1-2 weeks after the procedure
You will need to refrain from exercise for 6-8 weeks
You will need to wear a post-op bra for 6 weeks following the surgery
Avoid sleeping on your stomach during the recovery process, as this will place pressure on the implants
How soon will I see results?
You will start to notice visible results around one month after your surgery, once the majority of the swelling and bruising has subsided. You can expect full recovery to take up to 12 months, at which point your implants will have fully settled into their new shape and position.
How painful is the surgery?
Patients of Top Surgery commonly experience some level of discomfort and pain in the weeks following their procedure. While thresholds for pain vary from person to person, you are likely to feel sharp, short-term discomfort which can be relieved with pain medication.
How old do you have to be to undergo Gender-Affirming Breast Augmentation?
The minimum age for any Top Surgery consultation or procedure with Pall Mall Medical is 18.
Can you undergo this without a referral?
It is best to discuss with our Patient Liaison in the first instance. In most cases we cannot proceed without a referral letter from a licensed UK Gender Specialist, as per WPATH and NHS service specifications.
